Alzheimer's Association Care Navigators are licensed social workers who meet with individuals who are affected by Alzheimer’s disease or related dementias. We typically meet with caregivers, but can also meet with individuals who are diagnosed within the early stages of the disease. Care Navigators assist clients in identifying priority areas of concern, problem-solving, and developing a care plan. Sessions are based on a solutions-focused approach, and typically range from 1-3 appointments.
There is a fee for this service, which is billable to some insurance plans. Financial assistance may be available. Please email carenav@alz.org for assistance.
