Dementia, by definition, impairs an individual’s memory, decision-making, reasoning, and/or other areas of cognition. Given the nature of these impairments, the involvement of a trusted care partner is critical in navigating medical appointments and evaluations and advocating during hospitalizations and other care transitions. Family members and other loved ones who take on the care partner role often lack the depth and breadth of knowledge needed to feel confident in these roles.
