The Pearl of Montclare

Alzheimer's Care

Pearl Healthcare is a premier facility located in the Montclare neighborhood of Chicago, just minutes from Community First Hospital and many additional renowned Chicago hospitals, offering specialized clinical programming catered towards patients in need of short-term post hospital rehabilitation as well as long term care.

Assisted LivingPearl Healthcare was founded on the basic recognition that no two people are the same. Our unique approach to post acute rehabilitation places the patient at the center, curating a plan of care that understands the specific needs of each individual. A perfect marriage of innovation, compassion and clinical excellence makes the Pearl experience second to none.

At The Pearl of Montclare, you have the benefit of personalized care – our staff knows and respects you. They treat you like family. You’re in a friendly place that feels like home. And you have the security of a continuum of care – you don’t have to move away if your health care needs change. We offer assisted living, memory care, Medicare-certified skilled nursing care, in-house rehabilitation services and short-term caregiver relief.

Hospice CareSkilled Care residents require assistance in all daily living activities. Our team of licensed medical professionals provides full-time care and services that include physical therapy, occupational therapy and/or speech pathology. This level of care has often been viewed as hospital-like or institutional. But there is a noticeable difference at The Pearl of Montclare. Visitors often tell us how much they appreciate the homelike environment and the intimate feel that comes from our smaller setting. Our therapy room is cheery and bright, overlooking the beautiful neighborhood. It’s not cold and sterile, nor is it crowded. It is a wonderful place to recuperate. Skilled Care does not have to be permanent. Utilizing the medical services available may enable residents to return to a more independent living situation or be discharged to a home setting.

Alzheimer's and Memory Care are dedicated to meeting the daily needs of individuals in the middle to late stages of Alzheimer's disease and other types of dementia. Our compassionate staff is sensitive to the individual needs of our memory impaired residents and is committed to preserving the dignity and uniqueness of each person, and making them feel at home. Specially trained in dementia care, our Memory Care staff will make sure that your loved one receives the personalized care they deserve in a safe, secure and nurturing home-like environment.

Whether his or her health requires sheltered care, intermediate care or Medicare-certified skilled nursing care, we want each resident to achieve the highest degree of independence possible. We are committed to helping them achieve their potential.
