JourneyCare offers community-based programs designed to increase individuals understanding of end-of-life issues and ensure that all community members have equitable and just access to culturally aware quality end-of-life care, when needed. We are proud to have a team of Community Health Workers (CHWs) serving the north, south, and west sides of Chicago. We understand that each community is unique, and our education offerings reflect and respect these end-of-life care concerns. Each CHW lives among the community they primarily serve. The CHW team offers free presentations and workshops on topics including:
· Hospice 101: What is hospice care? This program explains the basics of hospice care including, eligibility, insurance coverage, levels of care, and what hospice care looks like for patients and families. Presentations include information on hospice care in general as well as specifics to our services at JourneyCare.
· Palliative Care 101: What is palliative care? This session explains the basic of palliative care including, eligibility, insurance coverage, services offered, and the difference between hospice care and palliative care.
· The Five Wishes: Empowerment through preparation. Every adult should document their preferences for end-of-life care. Through conversation, we explain the importance of advanced directives and introduce participants to the Five Wishes. Learn the benefits of this legal document that guides you in documenting your wishes for medical care decisions, and your end-of-life social, emotional, and spiritual choices.
All presentations are free of charge, appropriate for training community-based professionals or their clientele and can be delivered in-person or via virtual platforms. Most presentations are available in English and Spanish, and we welcome opportunities to partner with translators for additional languages.
